Vision Screening appointments can be made at CAC & a referral form is provided when scheduling your appointment. \nParticipants MUST bring Photo ID & Agency Referral on the day of screening. \n  \nLa elegibilidad se limita a los residentes del condado de Montgomery por debajo del nivel federal de pobreza.
